食道癌拒絶抗原ペプチドの癌ワクチンとしての基礎及び臨床研究

研究概要

高度進行・再発食道癌及び諸進行癌患者を対象に説明と同意を得て腫瘍細胞を採取し、IL-2の存在下で自己末梢血リンパ球と混合培養して癌特異的キラーT細胞の誘導を行なった。培養開始後2週前後で、自己腫瘍細胞傷害性活性化リンパ球が誘導され、この活性化リンパ球中にはCD4-CD8+のキラーT、NK、LAK細胞の混在を認めた。また、キラーT細胞にはHLA class I拘束性が認められた。これらのキラーT細胞を用いて、遺伝子発現cloning法により新たな癌拒絶抗原遺伝子の同定を行った。既に、食道癌が保有するHLA-A24結合性の癌拒絡抗原遺伝子を20数種類同定しているが、今回新たに薬剤耐性遺伝子MRP3遺伝子のHLA-A24,-A2結合性ペプチドが2種類同定された。既に開発したHLA-A24拘束性癌抗原ペプチド14種類を用いてCTL precursor orientedペプチドワクチン療法の第I相臨床試験を、臨床試験安全性評価委員会/倫理委員会の承認を得て開始した。
現在までの成績では、食道癌は進行が速くペプチド特異的キラーT前駆細胞の増加を認める6回以上の投与以前にprogressive diseaseとなり、ペプチドのみで治療することは困難であった。この傾向は、胃癌や膵臓癌患者でも認められており、増殖が急遠な癌腫に対しては、新たな治療対策を立てなければならないと思われた。
