肝硬変症の形態的および機能的回復の可能性の検討-ラットモデルでの肝部分切除術の効果-

研究概要

肝硬変症の改善を目的として肝部分切除術(肝切)を施行し、再生肝臓が長期的に肝硬変から改善するかを形態的および機能的に検討した。
【方法】ラットでthioacetamideで肝硬変を作成した。正常ラット(NORM)と肝硬変ラット(LC)で70%肝切を施行し21日までの肝臓の変化を形態的および機能的に検討するとともに、LCの肝切群と単開腹群で術後60日の残存肝を比較した。形態的な指標としては肝重量比、PCNA陽性率で、機能的な指標としてはインドシアニングリーンの消失率(ICG-k)、aminopyrineのクリアランス(CLamp)を測定した。
【結果】肝重量比はNORMでは肝切により4.0%から1.6%に減少し21日で前値に戻った。LCでも減少したが21日では前値まで回復しなかった。PCNA陽性率はLCで遅れそのピーク値も低かった。ICG-k値はNORMでは肝切後2日で最低となり7日で回復した。LCでは5日で前値に復した。体重あたりのCLampはNORMでは肝切により低下し21日でも回復は遅れた。LCでは7日で回復した。肝重量あたりのCLampはNORMでは肝切により低下し21日でも回復は遅れた。LCでは低下することなく機能的再生が先行した。LCでは単開腹群と比較して肝切により長期的に機能的肝再生の促進傾向が認められた。
【結論】LCでは肝機能総量は低下していた。NORMは機能的肝再生は形態的再生に遅れた。LCでは形態的肝再生はNORMに比べ緩徐であり機能的肝再生が形態的肝再生に先行した。LCで単開腹群と比較して肝切により長期的に機能的肝再生の促進傾向が認められた。
