頸動脈動脈硬化巣の形態学的、生化学的分析による脳梗塞発症機序の検討

研究概要

粥状硬化性頸動脈狭窄症に対する頸動脈内膜剥離術(以下CEA)により摘出されたplaque内の過酸化脂質および酸化LDLに着目し検討した。前年度はCEAにより摘出されたplaqueの病理学的にmacrophageやlipid coreが厚い不安定なplaqueほど過酸化脂質が多く抽出されたことを報告したが、本年度は板部らによって開発された抗酸化LDLモノクロール抗体(DLH3)を使用して動脈硬化巣の酸化LDLの含量を測定し、それが血中の酸化LDL濃度とどう関連するかを検討した。また手術前に頚動脈撮影や頚部超音波エコーで狭窄率のみならずplaqueの形態を評価し、CEAで摘出されたplaque(n=44)の病理組織所見及びplaqueの酸化LDL量との関連性を検討した。その結果、頚動脈エコーで低輝度を示したplaqueはmacrophageを多く含み、かつ、lipid coreが厚く、plaque ruptureやfibrous capの菲薄化の頻度が高い不安定なplaque(n=20)であり、その酸化LDL含量は安定したplaqueより3.6倍高値を示した。また免疫組織学的にDLH3抗体で強い染色性と一致した。しかし、症候性と無症候性との間ではplaque中の酸化LDL値には有意差はなった。患者血中の酸化LDL値は頚動脈狭窄症例でコントロール群(n=17)より有意に高かった。とりわけ不安定なplaqueを有していた症例ではコントロールより1.5倍と有意に高い値を示した。以上より酸化LDLはplaqueの性状と関連し、特に血中酸化LDL値の測定は頸部エコー所見とともにCEA手術適応の良い指標になることが示唆された。
