細径顎関節鏡の開発による顎関節下関節腔鏡視診断の確立

研究概要	1.イエウサギ膝関節変形性関節症(OA)モデルの作成安定性が確保されている膝関節半月板切除術によるOAモデルを術後4週(第1群)、2週(第2群)、1週(第3群)にて作成した。 2.膝関節の関節鏡視平成12年10月に納入された細径硬性関節鏡システムを用いて、全群を対象に鏡視検査を行った。鏡視所見の記録には、DVテープを用いた。当初、全群を対象に関節鏡視所見と肉眼解剖所見、HE染色による組織学的所見を比較検討する予定であったが、関節鏡が組織内部ではなく関節腔内の表面構造に対する検査法であることからOA変化の有無に関する至適基準をSEMに変更した。 3.走査電子顕微鏡的検索全群を対象に走査電子顕微鏡(SEM)を用い、大腿骨外側顆被覆軟骨における表面構造の変化を観察した。 4.その他の画像検索全群を対象に非侵襲検査である軟X線画像撮影、MRI画像撮影を行い、放射線専門医による大腿骨外側顆のOA変化の有無判定を行った。 5.検査特性の算出 2〜4で得た記録をもとに、SEM所見を至適基準として、1)関節鏡視、2)軟X線画像検査、3)MRI画像検査のOAに対する診断精度を求めた。その結果、第1群では1)100%2)83.3%3)83.3%第2群では1)100%2)50%3)66.67%第3群では1)100%2)16.67%3)33.33%であった。以上より、今回開発した細径硬性関節鏡システムによる鏡視診断はごく初期のOA変化に対しても高い診断精度を有することが示唆された。今後、ヒト死体(解剖実習用遺体)の下関節腔の関節鏡視をおこない、肉眼解剖所見を至適基準として関節鏡の検査特性の検討をおこなう予定である。
