白血病における白血球インテグリンMaC-1を介する線溶機序

研究概要

今回、患者白血病細胞上でのMac-1の発現率と、DICの発症との関連について検討したので報告する。
1.成績:
(1)患者白血病細胞上のMac-1の発現率:急性白血病患者18例を対象として、Mac-1発現率を測定すると、ALLに比べてAMLの方が高く、特にAMixL、CML急性転化例で高値であり、M2やM4の中にも80%以上の陽性率を示す症例が認められた。
(2)DICとMac-1:対象患者の18例のうち、化学療法開始前にDICと診断された症例は3例、開始後にDICに移行した症例が5例あった(DIC群)。その他の10例は、DICの合併を認めなかった(非DIC群)。M3の2例を含むDIC群8例と非DIC群10例における白血病細胞上Mac-1発現率を比較すると、有意差を認めなかったがむしろ非合併例で高値であった。Mac-1発現率が90%と高値であったM4ではTAT、PICの増加はわずかで、FDP、D-dimer、フィブリノゲン、TAT、PICとの間には相関は認めなかった。
(3)白血病細胞上のCD14発現率:CD14はLPSレセプターとしての機能を有しており、今回同時に発現率を検討した。それぞれのCD14陽性率は、M2で2例、M4で1例高値を示した。予想に反して、臨床的にDICを合併しやすいM3では低値であった。DICの合併の有無で比較すると、両群間に有意差を認めなかった。
2.考察:
以上の結果より、白血病細胞上のMac-1を介した凝固活性化機序が、臨床的な凝固亢進状態を引き起こす可能性はきわめて少ないと考えられる。少なくとも、APLにおけるDICの発症には、Mac-1の関与よりも白血病細胞中のTFによる凝固活性化が重要な要因になっているといえよう。
