肺癌脳転移と癌性胸膜炎における血管新生の分子生物学的検討と新規治療法開発

研究概要

本年度は、申請者が確立した癌性胸水(胸膜炎)および脳転移モデルを用い、血管新生因子VEGFの関与を主に検討した。癌性胸水について検討するために、VEGF高発現性のヒト肺腺癌株PC14PE6と低発現性の扁平上皮肺癌株H226をヌードマウスの尾静脈(i.v.)または胸腔内(i.t)に接種した。投与経路に関わらず両株は肺または胸腔内に多数結節を形成したが、PC14PE6を接種したマウス(i.v.及びi.t.)にのみ大量の血性癌性胸水が形成された。次にH226にVEGF遺伝子導入を行い、VEGFを強発現するH226/V165を得た。H226/V165をitした場合大量の血性胸水が形成されたが、ivした場合肺転移は形成するものの胸水を形成しなかったことより、胸水形成には胸膜浸潤とVEGF高発現の2つのステップが必要であることが示された。さらに、PC14PE6を用いた(i.v.)胸水モデルにおいて、VEGF受容体阻害剤による治療で胸水発症が有意に抑制された。一方、腫瘍細胞を顕微鏡下にヌードマウスの頚動脈から接種した場合、PC14PE6は5週以内に肉眼的脳転移を形成したが、H226は12週後でも顕微鏡的転移を形成するのみであった。PC14PE6の脳転移巣ではVEGFが高発現され血管密度は低いものの拡張血管が多数見られたが、H226のそれではVEGF発現は低く拡張血管は見られなかった。さらに、VEGFアンチセンス導入によりPC14PE6の脳転移および拡張血管形成が抑制されたが、H226へのVEGF遺伝子導入は脳転移形成を促進しなかった。よって、脳転移も複数のステップより構成されており、VEGFは拡張血管形成を介して脳転移形成を制御していることが示された。以上より、肺癌の癌性胸膜炎と脳転移形成のいずれにもVEGFが重要な役割を果たしており、VEGFを分子標的とした治療の有効性が示唆された。
