新しい感染症、新生児TSS様発疹症の発症機序の基礎的研究

研究概要	我々は、新生児におけるMRSA感染症(NTED)に伴う諸症状の発症機序を解明し、予防及び治療法を確立するために、当大学病院の新生児室及び全国の幾つかの新生児科病棟と共同で、患児におけるスーパー抗原応答性T細胞の運命やスーパー抗原刺激に対する応答性を解析した。その結果以下の事実を解明した。 1)NTED患児及び、健康MRSA保菌児血清中の抗TSST-1抗体の濃度を比較した。その結果NTED患児の方が抗TSST-1抗体の上昇が無く健康MRSA保菌児では上昇が見られた。この結果は健康MRSA保菌児のおいては、抗TSST-1抗体がNTEDの発症を抑制していることを示唆している。 2)NTED患児、健康MRSA保菌児から分離したMRSAのTSST-1産生について調べたところすべてのMRSAはTSST-1を産生していた。ほとんどすべてのMRSAがTSST-1を産生していることからメチシリン耐性遺伝子とTSST-1遺伝子との関係を検索中である。一方、ほとんどのMRSAはTSST-1とは別にSET6-15を保有している。この遺伝子産物の機能及びTSS発症との関係も同時に検索中である。 3)NTED患児のTSST-1応答性TCRVβ2陽性細胞の変動を調べたところ急性期前期に正常値以下に減少し抗原刺激初期のTCR発現の減少を生体内で初めて観察した。その後急性期には正常値の2倍から5倍まで増加、急性期後期では再び正常値以下に減少することがわかった。急性期後期に見られる減少はアポトーシスによるものと思われる。それに対して成人のTSS(2例観察)ではこのような急性期後期の減少は見られなかった。この違いは、新生児のTSSは軽傷で成人のTSSは重傷化するという事実と関係があるのではないかと考え、現在この違いを解明すべく実験動物モデル等を作成し検討中である。
