神経ペプチドの血液脳関門通過性とペプチドトランスポーターの解析

研究概要	オレキシンAは視床下部で産生され、食欲刺激作用と覚醒刺激作用を有する神経ペプチドである。オレキシンの異常によって、睡眠発作を主徴とするナルコレプシーが発生することが報告されている。これまで脳においてのみ産生されると考えられていたが、前年までの検討で、オレキシンAが消化管の神経叢の神経節細胞、消化管上皮の内分泌細胞や膵臓のランデルハンス氏島等の末梢組織からの産生・分泌が明らかになった。本年度は、以下の検討を行った 1.培養ウシ脳微小血管内皮細胞を用いたオレキシンAの取込みの検討:末梢組織由来のオレキシンAが血液脳関門を通過して脳に作用しているかどうか明らかにするために、培養ウシ脳微小血管内皮細胞を用いて、オレキシンAが血管内皮細胞に取込まれるか否か検討した。合成オレキシンAあるいは、125lでラベルしたオレキシンAを培養液中に加え培養し、細胞への取込みを検討したが、有意の取込みはみられなかった。 2.時無呼吸症候群の患者血中オレキシンA濃度の検討:睡眠時無呼吸症候群の患者156例を対象にして、血中オレキシンA濃度を検討したところ、軽症では軽度の上昇がみられたが、重症になるにつれ、有意な低下が見られた。睡眠時無呼吸症候群の患者の重症例では、視床下部オレキシンニューロンの活動性が低下しており、それが血中オレキシンA濃度低下に反映している可能性が示唆された。血中オレキシンAは、ナルコレプシーでは有意の変化は無かったが、睡眠時無呼吸症候群では、重症化するにつれ低値となった。脳内から血中へのペプチドトランスポーターの存在が示唆されたが、血管内皮細胞を用いた検討ではその存在は実証できなかった。ナルコレプシーと睡眠時無呼吸症候群の結果の差異の理由の解明も今後の検討が待たれる。
