潰瘍性大腸炎における粘膜微細環境の変化と遺伝子異常の蓄積:酸化ストレスによる発癌

研究概要

潰瘍性大腸炎(UC)からの発癌過程においては炎症の繰り返しに伴う酸化的ストレスにより上皮・間質細胞相互に遺伝子異常の蓄積があることを証明すべく、 1)UC再生大腸粘膜、UC関連異型病変dysplasia及び癌をフォルマリン固定・パラフィン切片上で、microdissection法により単一cryptの上皮細胞と隣接する間質細胞を分離採取・DNA抽出し、上皮細胞の分画よりp53蛋白をコードする全塩基exon2-11の変異の有無をPCR-direct sequencingによって調べた。その結果、UCの長期罹患例では再生粘膜の上皮細胞の一部には既にp53変異が生じており、low-grade dysplasiaではpolyclonalなp53変異がみとめられ、high-grade dysplasia, carcinomaではmonoclonalなp53変異に移行していることが判明した。(Submitted to J Pathol) 2)mild periodic acid Schiff染色(mPAS)によるnon-ο-acetylated epithelial sialoglycoproteinをoat^a/oat^b heterozygotesで同定することにより、mPAS陽性crypt/10^4 cryptsを数えてmutated cryptを算定することにより、UC患者では罹病期間に相関してmutated cryptsが増加することが判明した。(Revised manuscript, submitted to Cancer Res)
以上より、UCでは酸化的ストレスにより大腸粘膜上皮細胞の遺伝子変異が蓄積されることが判明した。次年度には間質細胞の変異の有無をLOH・microsatellite instabilityの検索によって明らかにし、間質細胞の変異も上皮細胞の癌化に関与していることを示す。
