ストローマ細胞との接着による急性白血病細胞の抗癌剤耐性誘導機序の解明

研究概要

急性骨髄性白血病(AML)の再発が骨髄から起こる事実をもとにして、癌細胞は正常細胞と同様に、組織ではアポトーシス(アノイキス)を起こし難くなるという仮説を立てて研究を行った。
その結果、
(1)患者AML細胞は、その細胞表面に発現しているβ1-インテグリンを介して、骨髄ストローマ細胞の細胞外マトリックス(ECM)であるフィブロネクチンと接着する事により、アノイキスを起こし難くなり、更に抗癌剤(DNR,AraC)によるアポトーシスから回避しやすくなった。
(2)アノイキス回避の機序について検討したところ、β1-インテグリンであるVLA4を介したフィブロネクチンとの接着により、AML細胞内のBcl-2の発現が亢進し、caspase9とcaspase3の活性化が抑制された。
(3)VLA4を発現しているAML患者群の化学療法後の予後が、VLA4陰性AML患者群のそれに比べて有意に不良であることも見い出した。即ち、24人のde novo AML患者の内、15人のVLA4陽性患者の完全寛解率は60%であり、9人のVLA4陰性患者のそれ(100%)に比べて有意に不良であった(P=0.028)。Kaplan-Meier法を用いた予測5年生存率も、VLA4陽性患者群では25%であり、VLA4陰性患者群のそれ(100%)に比べて有意に低値を示した(P=0.02)。
以上の結果から、
VLA4の発現が化学療法施行後のinitial drug resistanceと骨髄の微少残存病変の原因となり、再発に寄与する事が示唆された。
