タイ国寄生虫感染関連胆道がんの症例・対照研究による薬物代謝酵素遺伝子多型

研究概要	胆道がんは、早期発見が困難であり予後は極めて不良であり、その研究が重要でありながら進んでいない現状が有る。これには、欧米では胆道がんの罹患率が低いことや、診断時にはすでに進行がんであり手術が出来なくて検体が得にくいこと等による。タイ王国東北部の肝臓がんの年齢調整罹患率は男性87.5、女性37.2(バンコクではそれぞれ11.0および3.4)(Cancerin Thailand Vol. II,1992-1994,IARC Technical Report No. 34)と世界一高率である。また、同地方の肝臓がんの多くが肝内胆管がん(cholangiocarcinoma)であるとされている。疫学研究及び動物実験から肝吸虫Opisthorchis viverrini感染と食品由来のニトロサミンの生成が要因として疑われている。我々は、胆道がんの発生に薬物代謝酵素の遺伝的多型による酵素活性の差が胆道がん感受性の個体差を規定しているとの作業仮説を立て症例・対照研究を開始している。ニトロサミンの代謝的活性化に働く分子種CYP2A6および各種発がん剤の解毒に関係する酵素のglutathione S-transferase(GST)等の遺伝的多型を調べるため、タイ王国国立がん研究所の分子疫学者との共同で血液サンプルの採取を開始している。特に対照者は、症例と同じ地域に住んでいる性・年齢がマッチした方を選ぶことが本研究の最も重要な点の一つである。また、対照者には、血液中の腫瘍マーカー等に加えてその地域の中央病院に相当するProvincial Hospitalに出向いて超音波検査を受けてもらい、肝腫瘍を持たないことをチェックすることで厳密性を増すことにしている。対照者として採血した後、肝臓に腫瘍が発見され症例に再分類された例もある。現在までに、症例102例、対照89例の血液を集めており、近く結果を得ることが出来ると考えている。
