ヒト癌におけるムチン遺伝子ならびに新しい接着因子の発現と予後との臨床疫学的研究

研究概要

ムチンに関しては、種々の臓器の腫瘍のタイプによって大きく異るMUC2(腸型分泌ムチン)の遺伝子発現へのDNAメチル化の関与の検索を開始し、膵癌細胞株(MUC2を発現しないPANC-1と、MUC2を発現するBxPC-3)でのMUC2発現がDNAメチル化の有無によって規定されている可能性をmethylation specific PCR法により明らかにした。さらに、外科切除標本からのマイクロディセクションにおけるメチル化の検索についての実験も開始し、微量の組織からでも、培養細胞に用いたPCRプロトコールでDNAが増幅できることが確認できた。一方、膵胆管系の腫瘍におけるMUC1(膜結合型ムチン)、MUC2、MUC4(気道型ムチン)の発現を加味した新しい分類法を提唱し、膵腫瘍では、浸潤性膵管癌、膵管内乳頭粘液性腫瘍・暗細胞型ならびに膵管内乳頭粘液性腫瘍・明細胞型におけるムチン発現様式の顕著な違いを、粘液産生性胆管腫瘍では、胆管粘液性嚢胞腺癌と胆管内乳頭粘液性腫瘍とのムチン発現の差異を明確にした。また、原発性肺腺癌におけるMUC1とSurfactant apoprotein A(SP-A)の発現の組み合わせ評価を行い、MUC1/DF3>SP-A群が有意に術後再発率が高く予後も不良であることを明らかにした。さらに、唾液腺多形腺腫の再発予測因子としてMUC1が有用である可能性を示した。胃MALTリンパ腫の濾胞樹状細胞におけるMUC1発現の意義や、大腸癌組織におけるGal 3-O-sulfotransferase-2の低下による硫酸化ムチンの発現量の減少等をも明らかにした。
新しい接着因子に関しては、皮膚の扁平上皮癌と基底細胞癌の鑑別診断へのトロンボモデュリンの免疫染色の有用性を明確にし、また、胃癌においてガレクチン-4の発現が予後良好因子であることを見出した。
