悪性リンパ腫の地理病理学的、臨床病理学的研究

研究概要	悪性リンパ腫は、臨床病理学的、地理病学的および生物学的特性において極めて多様性に富む。ごく最近、新たな悪性リンパ腫分類、WHO分類が公刊された。これに基づく欧亜諸国との地理病理学的観点からのリンパ腫の比較検討は、本邦リンパ腫の特性の把握に必須である。我々は、既に本邦悪性リンパ腫症例3194例のWHO分類に基ずく亜型頻度を報告した。さらに今年度、欧州およびアジア諸国との比較検討を目的としてHans Konrad Muller-Hermelink教授(University of Wuerzburg, Germany)、Chul Woo Kim教授(Seoul University, Korea)らと協議し、診断基準の検証を計ると同時に共同研究の開始について一定の合意を得るに至った。また、分子病態に基づく悪性リンパ腫の臨床病理学的解析では、本邦における主要な血液疾患治療施設との多施設共同研究を推し進めることにより多くの成果が得られた。特にマントル細胞リンパ腫におけるcyclinD1、未分化大細胞リンパ腫におけるALK、CD56の臨床病理学的意義を明らかにすることが出来た。臨床経過、治療反応性を全国規模で集積することにより、それらが有意な生物学的予後予測因子であるとの確定的データを得、論文として公表するに至った。今後さらに疾患単位ごとにデータの集積を計ると共に欧亜諸国との比較検討を試みる予定である。また検討の過程で幾つかの新たな"疾患単位"が認識されつつあり、併せて解析を進める予定である。
