研究概要

歯周病原性細菌をPCR法により簡便にその種類を定量し、モニターしていくことで、病態を正確に把握し、治療方針の決定、予後の判定に有効であることが示された。またDNAマイクロアレイを応用することにより病原因子の遺伝子レベルでのモニタリングにより、より詳細な歯周病の活動度を診査する可能性も示唆された。一方で疾病の感受性診断についてもいくつかの有用な診断法が開発された。FcγR遺伝子はIL-1などの炎症性サイトカイン産生を制御していることから、その多型性の検査は歯周炎による生体侵襲に対する応答性に影響し、個人に応じたオーダーメイド治療の開発に役立つと考えられる。こうした生体側の反応性については炎症関連因子について詳細な考察がなされており、炎症関連因子のプロファイル解析を行い、それらをターゲットとした新たな治療法の開発の可能性も秘めている。こうした炎症関連因子のプロファイル解析については組織検体を用いたものから、簡便な唾液や歯肉溝浸出液からのスクリーニング、プロテインチップを用いた方法を開発している。さらにチェアサイドで簡便に行える検査法としてグリコサミノグリカンを指標とした歯槽骨活性検査法を開発した。さらに歯肉溝浸出液からのカルプロテクチンの測定は歯周病の活動度を反映していることから、現在の状態を正確に把握する手段として非常に有用であることが明らかとなった。呼気中の揮発性硫黄化合物が歯周病と密接に関係していることも明らかとされ、口腔内気体分析による歯周病診断法も開発された。歯周組織破壊の重要なファクターに外傷性咬合があるが、固定法を用いて外傷性咬合を検知する方法を開発した。また歯周炎が生体に対してどの程度の炎症状態であるか、また虚血性疾患を始めとする全身疾患へのリスクの予測に高感度CRPの有用性が示された。
