急性膵炎の後期重症化機構におけるCARSの関与に関する研究

研究概要	サイトカイン及びサイトカイン阻害物質の血中動態は急性膵炎の重症化と関連している。重症急性膵炎では、おそらくその高度の組織破壊の影響で、多くのサイトカインの産生が誘導されて、サイトカインストームとも言うべき状態となる。サイトカインは本来は生体防御反応として産生されるが、過剰に産生されると、臓器障害や感染防御能の低下など、生体障害性の反応が惹起される側面を有する。急性膵炎の後期重症化の背景にはCARSに伴う易感染性がある。この生体防御反応は生物の生存に不可欠のものであるため、精巧なバックアップシステム、フェイルセーフシステム、フィードバックシステムを有している。そして局所のSIRSに対するフィードバックシステムの作動によって全身は逆にCARS状態となる。このとき局所のSIRSが大きいほど、全身のCARSは大きくなる。LISISと呼ぶべき病態である。両者が平衡を保っている状態であるために、片方を抑える治療(例えば炎症性サイトカインの阻害療法)の効果が発揮されないのではないか、と考えられる。本研究の結果、急性膵炎の診療においては、SIRSとCARSの両側面への対策が肝要であることが明らかとなった。 SIRSやCARSの概念の登場とともに、侵襲反応の種々の側面が明らかとなってきたが、未だほんの一側面をとらえているにすぎないと思われる。生体が組織する侵襲反応の全体像を把握するためには、さらなる探求が必要である。
