研究概要

「膵癌の神経浸潤機構の解明」についてこれまでに我々が得た結果を以下に報告する。
1.臨床検体における免疫染色では、膵内神経叢、腹腔神経叢においてGDNF、RETの発現を認めており、膵および膵周囲神経組織からのGDNF分泌を証明した。
2.GDNF-RETを介したシグナル伝達経路における転写因子NF-κBの役割を検討した。
(1)Proteasome inhibitorであるMG-132の作用により、膵癌細胞のproliferationはMG-132の濃度依存性に減弱した。
(2)細胞外マトリックスへの膵癌細胞の浸潤能とNF-κB活性は、GDNF刺激により亢進した。MG-132の存在下、あるいはI-κBαM vectorを遺伝子導入した状態では、膵癌細胞の浸潤能とNF-κB活性は有意に抑制され、GDNF刺激によっても抑制されたままであった。
(3)GDNFは、Phosphatidylinositol 3-kinase(PI3-K)が関与するシグナル伝達経路を介してMatrix metalloproteinase-9の発現・活性を亢進させることによっても、膵癌細胞の運動・浸潤能を亢進させている可能性が示唆された。
これらの結果は、膵癌の好神経進展性を考える上で、新しい重要な考え方であり、膵癌の神経浸潤の機序解明に大きな進展をもたらすものと考える。特に、NF-κBの活性を遺伝子導入により抑制、さらには膵癌細胞の浸潤能までも抑制できたことは、根治が極めて難しい膵癌の今後の補助療法治療に、遺伝子治療が大きな効果をもたらす可能性があることが示唆されたものと考える。
