放射線誘発大腸癌における遺伝子変異および素因に関する研究

研究概要	平成15年度は、以下の項目について検討あるいは準備を進めた。 1.昨年度に引き続き、MLH1ノックアウトマウスを用いてHNPCC患者における放射線治療の安全性について検討を行った。急性障害の指標として腸管のcryptにおけるアポトーシスへの影響を観察し、また晩期障害として二次発癌や皮膚炎などの有害事象の発生について検討した。生後24週時点での解剖所見では、+/+群では消化管腫瘍の発生は認められなかったが、ヒトHNPCCモデルとなる+/-マウスにおいて、メスでは10%以上の症例に小腸を中心とした腸管に腫瘍の発生がみられた。しかし、対照群と比較して照射群で腫瘍の発生率の増加は認められなかった。今後、さらに生後48週の解剖結果ともあわせて結論を出す予定であるが、中間解析の結果では、モデルマウスに関する限りMLH1遺伝子変異マウス(+/-)は野生型と比較して、消化管腫瘍の自然発生は増加しているものの、照射により腫瘍の発生率は増加しないと考えられた。昨年度の大腸癌細胞株を用いた放射線感受性に関する検討で、MSI陽性大腸癌細胞株は陰性株に比べてコロニー形成試験では放射線感受性が高いことが判明した(Arai M.et al.2004 in submission)ので、本年度はさらに本マウスに発生した腫瘍を指標として放射線感受性について解析する。 2.放射線感受性の個体差に関する検討は、現在検体を収集している段階である。放射線感受性に関係すると推測される遺伝子群を選定し、これらの遺伝子のSNPを解析する。対象は、子宮頸癌治療のため放射線照射により直腸炎などの有害事象が発生した有害事象群と明らかな副作用の認められなかった対照群である。有害事象群11名の採血とEBウイルスによるリンパ球の株化を行った。本年度はこれと併行して対照群の検体収集も行い、多型解析を開始して統計学的な検討まで終了する予定である。
