ヒトT細胞白血病ウイルスI型感染シェーグレン症候群発症予知の遺伝子診断と発症阻止

研究概要	Sjogren症候群患者の発症におけるIL-10遺伝子多型の関与を明らかにすることを目的として、Sjogren症候群患者57例および健常人107例を対象として、IL-10プロモーター領域(-1082A/G、-819C/T、-592A/C)遺伝子多型を、ゲノムDNAを用いたPCRの後、ABI3100による直接シークエンス法またはRFLP法で解析した。結果、各IL-10遺伝子多型のallele頻度は、Sjogren症候群患者と健常人の間に有意差を認めなかったが、ACC haplotype (-1082A、-819C、-592C)の頻度は、健常人に比べSjogren症候群患者において有意に低頻度であった(29% vs. 18%、 p=0.037)。また、ACC carrierの頻度もSjogren症候群患者において有意に低頻度であった(51% vs. 33%、 p=0.033)。さらに、40歳以下で発症したSjogren症候群患者は40歳以上で発症した患者よりさらに、ACC haplotypeの頻度が有意に低頻度であった(4% vs. 28%、p=0.012)。HTLV-1抗体陽性者はACC haplotypeの頻度がHTLV-1抗体陰性者に比べ低い傾向があったが、有意差はなかった。その他、IL-10遺伝子多型とSS-A抗体陽性およびSS-B抗体陽性、抗核抗体陽性、乾燥症状、Schirmer test, Saxon test異常においては相関はみられなかった。以上のことから、IL-10遺伝子プロモーター領域における遺伝子多型が、Sjogren症候群の発症に関与することが示唆された。
