IgG4関連膵炎(自己免疫性膵炎)の病態の解明と特異的診断法の確立

研究概要	本疾患は高齢者男性に好発し、既存の自己抗体の出現頻度が少ないことが明らかとなった。シェーグレン症候群時に認められるSS-A, SS-B抗体については全例陰性であった。また、膵管像の検討より本疾患の病変範囲は必ずしも膵全体におよぶわけではなく、限局性に留まる例も約半数に認め、瀰漫性に移行する症例、自然軽快する症例も認めた。本疾患の画像所見は病態に応じて多様に変化することが明かとなった.また、後腹膜線維症の合併例の検討より本疾患はmultifocal idiopathic fibrosclerosis(MIF)に包括される概念と考えられ、IgG4がその病態に関与していると考えられた。しかし、膵臓に首座を置く病態も明らかに存在し、これらと他臓器病変合併例とはαフォドリンに対する自己抗体の出現様式より区別される可能性が示唆された。本疾患でDRB1^0405-DQB1^0401 haplotypeの頻度が有為に高く、classII抗原でpresentationされる特定の抗原ペプチドに対する免疫応答が発症の引き金になっていると考えられた,さらにB54とはリンクしていないことより自己免疫性肝炎とは免疫遺伝学的背景が異なっていると考えられた。本研究より自己免疫性膵炎では特定の抗原に対する自己抗体が存在すると考えられ、今後さらに研究を進めていく必要があると考えられた。膵臓の全蛋白質をSDS-電気泳動し、複数の自己免疫性膵炎の患者血清と、複数の正常人血清を用いてウエスタンブロテイングした結果、患者血清にcarbonic anhydlraseとは異なる、患者特異的な25kDaのバンドを認めた。この蛋白は不溶性の画分に存在した。今後、二次元電気泳動で単一のスポットが同定出来るまで精製を進め、その後ゲルからスポットを切り出し、アミノ酸配列を決定する予定である。
