III期非小細胞肺癌に対する新規抗癌剤+プラチナと放射線併用の比較試験

研究概要	本研究は切除不能限局型進行非小細胞における新規治療法の有効性を検討する無作為化比較試験である。新規治療法としてはイリリノテカン+カルボプラチン(IC)あるいはタキソール+カルボプラチン(TC)を週1回投与し胸部放射線(TRT)1日1回2 Gyを6週(60Gy)と同時併用する2群、対照群はマイトマイシン+ビンデシン+シスプラチン(MVP)の4週毎2サイクル投与とTRT1日1回2Gy、3週を1、2サイクル目に併用するものである。いずれの群も放射線化学療法終了後にそれぞれ2剤併用の通常投与量を2サイクル追加する。対象は未治療の切除不能IIIA/B期非小細胞肺癌で、75歳以下、PSO、1、測定可能病変を有し、骨髄機能および主要臓器機能が保持され、文書でインフォームド・コンセントの得られた症例である。プライマリー・エンドポイントは5年生存率で10%の向上を期待し、目標症例数は450症例とした。2000年10月より試験を開始し、2003年12月末時点で275例が登録された。275例の内訳はMVP群92例(男79例、女13例、IIIA28例、IIIB64例、年齢31〜74歳)IC群91例(男78例、女13例、IIIA27例、IIIB64例、年齢30〜75歳)、TC群92例(男79例、女13例、IIIA27例、IIIB65例、年齢38〜74歳)で3群間の背景因子はバランス良く配分されている。これまでに治療関連死は5例(MVP群0、IC群3例、TC群2例)報告されているが、3群とも耐用可能な治療法として試験は継続している。300例が集積された時点で中間解析を実施し試験の経緯を判断することにしている。
