アジアにおける母子の感染症の分子疫学的研究(血液感染および経口感染ウイルスを中心にして)

研究概要

B型肝炎、下痢症ウイルス、エイズに関して主に行った。(1)HBVは6つの主要なゲノタイプがあり、HBV感染でみられる肝病態はゲノタイプによって異なる。型特異的プライマーをデザインし、PCRにて型別をした。さらに遺伝子解析により分子系統樹を作った。スペインA型(57%)、米国A型およびC型(各45%)、韓国B型(53%)、ベトナムC型(57%)日本C型(75%)、中国C型(79%)、ミャンマーC型(95%)、ロシアD型(62%)、エジプトD型(100%)、ガーナE型(75%)、ボリビアE型(75%)であった。この方法でゲノタイプが分かれた。今後、病原性、母子感染率、HBV変異株の出現、治療効果などの相違点について解析したい。(2)下痢症ウイルスにはロタウイルス(RV),カリシウイルス(CV)が重要で、ワクチン開発・集団発症の予防のためには分子疫学的解析が必要である。RVは日本では1型中心であるが地域によっては9型が50%を越えていた;ベトナムでは、1型69%;タイでは、1型とともに9型が多く見られた;中国では1型が60-90%。カリシウイルスは日本以外では調査が進んでない。アデノウイルスは、41型が多いが、わずかながら40型、その他の型が見られた。(3)HIVの母子感染はタイでの調査に関し、バンコク第一地区における妊婦のHIV感染頻度はこの5年間2%位で横ばいである。また年齢をみると20-24歳が41%でピークを示していたが、小中学卒59%で、高卒が22%であった。主婦が40.8%、商売が15.5%、雇用27.7%であった。HIV感染が証明された時点で初産が42%、2児目が25.4%であった。結婚前あるいは後に感染を受けていた。RT-PCRによるサブタイプ別のプラマーをB, E以外についても検討し、ほぼすべてのサブタイプに満足できるプラマーを設定した。ブラジルの検体から既存のサブタイプとは境界型と考えられるものが1つ見出された。
