アジアにおける母子の感染症の分子疫学的研究(血液感染および経口感染ウイルスを中心にして)

研究概要	小児の感染症に関しアジアにおける分子疫学を行った。特に下痢症ウイルスと肝炎ウイルスに注目した。ロタウイルスに関しては、日本ではA群1型が急速に少なくなっており80%から40%になった。それに代わって2型、9型が増えてきた。日本での流行に変化が見られた。タイ国チェンマイでは引き続き9型のロタウイルスが圧倒的に多いことがわかった。また、ベトナムでは1型が主流ではあるが2,9型がわずかに見られた。この傾向は続いている。中国雲南省および北京の検体は1型が多く見られた。多種の下痢症ウイルスの遺伝子増幅による検出法を完成した。即ちAセットではロタウイルスA, B, C群とアデノウイルスを調べ、BセットではノロウイルスI, II、サポウイルス、アストロウイルスを検出できる。また,陽性の場合には、それぞれのウイルスでnested PCRおよび制限酵素を用いて、あるいは遺伝子解析でgenotypeを決定した。腸管アデノウイルスの頻度は5%前後であるが、41型を各国で見出した。ノロウイルスに関しては、Lordsdale近似株が各国で多かった。しかし稀な遺伝子型も見出された。数%のサポウイルスを認めた。サポウイルスで極めて稀有な遺伝子配列を有する株を見出し,全塩基配列の決定を行っている。A型肝炎あるいはE型肝炎ならびにSARSコロナウイルス関連の遺伝子増幅法での診断を試みたが陽性はなかった。 B型肝炎ウイルス(HBV)については、ベトナムに流行しているHBVの特性を解明するためにCPとPC領域の変異について解析した。PCの変異(G1896A)はゲノタイプBで50%、ゲノタイプCで4.3%の頻度で検出された(P=0.0001)。これに対しCP変異は肝硬変患者から分離されたHBVで高率に出現した(P=0.05)。血中HBV DNA量には、CPとPC変異の両者間で差を認めなかった。以上の成績は、ベトナムで流行しているHBVはCPとPC領域に高頻度に変異を伴っており、特にゲノタイプCにおける高いCP変異の出現は、同国における肝癌・肝硬変を含む重症慢性肝疾患の高い発生率と関連していると思われた。昨年度HIVのサブタイプの診断をより迅速に簡便に行うために各サブタイプに特有なプライマーをgp120領域で設定した。このプライマーを用いて臨床診断を行っている。トリインフルエンザやSARSコロナウイルスなど、新しいウイルスのアジアにおける流行が見られる。分子疫学の必要性がさらに増した。この点も含めてさらに検討していきたい。
