Toll様受容体遺伝子多型によるH.pylori応答性単球動態と胃粘膜疾患関連性

研究概要	ヒトTLR4(Toll-like recetor 4)の遺伝子多型について。本研究は、慶應義塾大学医学部倫理委員会の承認(受付番号13-93;2002年4月2日付)を得て実施された。インフォームド・コンセントを得られたH.pylori陽性の慢性胃炎患者のうち、定期的な上部消化管内視鏡検査をうけている患者を対象として、採血した血液10mlからDNAを抽出し、遺伝子多型解析に供した。TLR4遺伝子多型については、最近報告された日本人のSNP(single nucleotide polymorphism)のJSNPデータベースに収載されたSNPとTLR4ゲノム配列とを比較し、侯補SNP部位を12ヶ所、選定した。これらの侯補SNP部位を増幅するPCRプライマーを設計し正常群の患者のゲノムDNA等濃度混合物を直接シーケンシング法により解析し、ヘテロ接合率の高いSNPをプロモーター部位に1ヶ所GenBankTLR4ゲノム配列AF177765の第2209番目の核酸配列がAからGへの置換、イントロン2ヶ所に1ヶ所SNPGenBankTLR4ゲノム配列AF177765の第11771番目の核酸配列がGからTへの置換を同定した。それぞれのSNPについて熱変性高速液体クロマトグラフィー法によりタイピングを行った。2つのSNPのタイピング結果から、両者がきわめて強い連鎖不平衡にあることが示された。2209Aアレルおよび2209Gアレルの頻度はそれぞれ71%と29%であった。2209Aホモ接合体は47%、2209A/Gヘテロ接合体47%・2209Gホモ接合体は5%で、両アレルの間にHardy-Weinberg平衡が成立していた。慢性胃炎患者群38名をCorpus predominant gastritis(14例)とAntrum predominant gastritis(24例)の2群にわけ、TLR4多型との関連を検討した。結果として、胃炎の進展のtopographyで分類した2群間では,TLR4遺伝子多型に有意差を認めなかった(p=0.52)。
