冠動脈疾患における免疫系の関与に関する研究

研究概要

冠動脈内プラーク破綻の機序はまだ明らかではないが、冠動脈プラーク内にT-cellやマクロファージなどの炎症性細胞の集積があり、そこから冠動脈プラーク破綻が生じる可能性が強く示唆されている。それと同時に循環血液中でもモノサイトやリンパ球が活性化されているという報告がある。本研究においては、フローサイトメーターを用いてT-cellに刺激を与えてinterferon-γ (IFN-γ)とinterleukin-4 (IL-4)の産生を指標にし、不安定狭心患者のT helper cellのTh1 cell(細胞性免疫担当)とTh2 cell(液性免疫担当)への活性化について検討した。不安定狭心症患者からの採血で得られたT-cellは対照患者や安定労作狭心症患者から得られたT-cellにくらべIL-4の産生能の亢進はなかったがIFN-γの産生能が亢進していた。これにより不安定狭心症患者ではTh2 cellへの活性化はあまり強くなく、Th1 cellへ活性化が強いことがわかり、不安定狭心症の発症メカニズムとの関連が示された。また、冠動脈硬化の程度とTh1 cellへの活性化の程度にも相関があるようである。さらに冠攣縮性狭心症は不安定狭心症のひとつの原因でもあるが、冠攣縮性狭心症患者からの採血で得られたT-cellにおいてもIL-4の産生能亢進はなかったが、IFN-γの産生能が亢進していた。これにより冠攣縮性狭心症の病態にもTh1 cellの関与があることが示されたことになる。現在、動脈硬化の局所では内皮細胞、平滑筋細胞、マクロファージがj活性化されCD40 ligandというT-cellの表面マーカーCD40の受容体について検討し、動脈硬化の局所の細胞の活性化の程度を不安定狭心症患者および冠攣縮性狭心症患者において検討している。さらに経皮的経皮的冠動脈形成術前後のTh1 cellとTh2 cellへの活性化について検討している。今後これらIFN-γ、IL-4、CD40 ligandについて患者の心事故発症との関連も検討していく予定である。
