小児期にみられる肝炎後再生不良性貧血における造血障害機序の研究

研究概要

肝炎後再生不良性貧血(再不貧)に先行する肝炎の多くは激症型であり、その原因ウィルスの同定が試みられていきた。当科のこれまでの検討でもnon Anon BnonC型であり、新たに肝炎ウイルスと同定されたG型肝炎ウィルスやTTVウィルスの関与も否定された。
本年度になってこれまで我が国では存在しないと考えられていたE型肝炎ウィルスが、一部の激症型肝炎の原因となりうることが判明した。E型肝炎の感染経路は経口感染であり、患得援護再不貧の患者の多くは輸血歴がないことから、原因ウィルスの候補となりうる。肝炎後再不貧(n=16)、特発性再不貧(n=7)、正常人コントロール(n=7)のHEVlgM抗体、HEVlgG抗体を検討した。あわせてパルボB19lgG抗体、B19lgM抗体についても検討した。
それぞれの抗体の陽性率は、HEVlgM(肝炎後:0/16、特発性0/7、正常人0/7)、HEVlgG(肝炎後:0/16、特発性0/7、正常人0/7)、パルボB19lgM抗体(肝炎後:0/16、特発性2/7、正常人0/7),パルボB19lgG抗体(肝炎後:11/16、特発性6/7、正常人2/7)であった。
今回の結果からE型肝炎ウィルスが肝炎後再不貧に先行する肝炎の原因ウィルスである可能性は否定できると考えられた。
パルボB19については、肝炎後再不貧患者において抗体保有率は高いが特発性再不貧患者も同様であり。原因ウィルスとは考えにくい。肝炎後再不貧に先行する肝炎の原因は不明であり、さらなる検討が必要である。
