新しい糖尿病発症機序の解明:血中過酸化脂質の増加による膵β細胞障害について

研究概要

膵ラ氏島では過酸化脂質に対する処理能が他臓器に比して著しく低下していることが認められている。したがって酸化ストレスに対する防御機構の脆弱なことが細胞機能障害に関連し、インスリン分泌能が変化すると想定される。本研究の目的は膵β細胞に対する過酸化脂質の担体としての酸化リポ蛋白によるインスリン分泌障害の関係を遺伝子変化から明らかにすることである。1年目の検討ではハムスター由来膵β細胞の細胞株であるHit-T15細胞に対して酸化LDLを添加培養することによりインスリン分泌能が低下することを明らかにした。これはLDLの酸化変性に伴った変化であった。本年度はこのようなoxLDLの作用がインスリン遺伝子発現の変化によるものかどうかを検討した。その結果、oxLDLはHit-T15細胞のプロインスリンmRNAの発現量を用量依存性にまた時間依存性に減少させることが明らかとなった。この変化はnLDL、AcLDLの添加培養では認められないことからoxLDLに特有な変化と考えられた。このようなoxLDLがHit-T15細胞内に取り込まれるか否かを検討すると、DiI標識oxLDLは細胞内に取り込まれることを認めた。したがってoxLDLはHit-T15細胞内に摂取された後、インスリン遺伝子mRNAの発現を低下させてインスリン分泌能を阻害することが示された。このようなoxLDL/過酸化脂質のβ細胞に対する直接作用を検討した成績は初めてである。以上のことから「高脂血症-oxLDL/過酸化脂質-膵β細胞機能障害」といった糖尿病発症経路の存在が示唆された。このようなβ細胞に対する代謝障害経路は新たな糖尿病発症機序を想定させるものであり、臨床面での解明が必要である。
