CD4陽性T細胞に認識される上皮性癌抗原遺伝子の同定と癌ワクチンへの応用

研究概要

1.久留米大学で進行中であるHLAクラスI分子に提示される抗原ペプチドを用いた癌ワクチンの臨床研究では、患者によっては投与された癌抗原ペプチド(9-mer,10-mer)に対するIgG抗体が著明に誘導される症例があることを観察している。生体内で抗原特異的IgGが誘導されるためには、抗体を産生するB細胞にサイトカインを供給するヘルパーT細胞の存在が示唆されるが、実際に9-merであるUBE2V 43-51ペプチドを投与された肺癌患者の末梢血リンパ球中に、UBE2V 43-51ペプチドをHLA-DRB1^*0403拘束性に認識するCD4陽性T細胞の存在を確認した。また、UBE2V 43-51ペプチドは6番目にシステインを有しており、酸化によりシスチン化や2量体化されやすいが、UBE2V 43-51ペプチドを認識するCD4陽性T細胞は、UBE2V 43-51ペプチドを単量体としてのみでなく2量体としても認識することを証明した。(Harada M.et al.,J.Immunol 2004)
2.申請者は、再燃前立腺癌に対するワクチン療法確立のため、日本人に多いHLA-A^*2402分子に提示され前立腺癌に対する紳胞傷害性T細胞を誘導できるペプチド同定を試み、prostate-specific antigen(PSA)152-160,PSA 248-257ペプチドを同定した。(Harada M et al.,Prostate 2003)また、PSA 248-257ペプチドは、前立腺癌患者の約60%、健康人の約80%でこのペプチドに対するIgG抗体の存在が認められた。このことは、前立腺癌患者や健康人にPSA抗原を認識するCD4陽性T細胞が存在することを間接的に示唆していると考えられ、実際に、PSA 248-257ペプチドのワクチンを受け、このペプチドに対するIgG抗体が増加した前立腺癌の末梢血リンパ球よりPSA 248-257ペプチドを含むlong PSAペプチドを認識するCD4陽性T細胞を確立した。しかしながら、確立したCD4陽性T細胞は、投与されたPSA 248-257ペプチドを認識できないことから、癌ワクチンにより癌患者内でepitope spreadingが生じたことが示唆された。
