新規シスタチン様転移関連遺伝子CMAPを分子標的とした肝転移機構の解明

研究概要

我々はマウス肝高転移株で高発現しているCMAP遺伝子を同定し、同株へのCMAP antisense DNAの誘導が実験的肝転移能を抑制することを報告した。大腸癌肝転移症例でもCMAPが癌部で高発現していることも確認した。そこで、肝細胞癌(肝癌)でのCMAP発現を解析し、術後肝内再発予知マーカーとなり得るか否かを検討した。肝癌切除65例の癌部及び非癌部におけるCMAPのmRNA発現量をReal-time定量的PCR法にて測定した。発現量の補正には同時に測定したBeta-Actinを用いた。(検討1)CMAP発現と臨床病理学的因子(27因子)との相関を単変量・多変量解析にて検討した。(検討2)上記65例中、治癒切除で1年以上の詳細な経過観察が可能であった45例(他病死は除く)を対象に、CMAP発現と予後との相関を再発形式も含め検討した。その結果、(検討1)癌部CMAP発現量の中央値で2群に分け比較した。単変量解析では、CMAP高発現例で有意に(P<0.05)肝内転移陽生例が高頻度。非癌部CMAP高発現例では有意(P<0.05)にGOTが高値。多変量解析でもCMAP高発現は肝内転移に関する独立因子であった。(検討2)45例の術後経過観察期間中28例に再発を認めた。初回再発臓器は25例が肝内、3例が肝外(骨、肺)であった。単変量解析による無再発生存率の比較では、癌部CMAP高発現例に再発例が多く、再発形式別にみると、癌部CMAP高発現例は、有意に肝内再発率が高かった(P<0.05)。多変量解析の結果でも、癌部CMAP高発現はGOT高値と共に独立した肝内再発危険因子であった。以上より、肝癌におけるCMAP発現は肝内再発予知マーカーとなり得る。現在、CMAPを分子標的とした新しい肝転移再発制御法の開発のため、CMAP遺伝子が肝転移に関与するメカニズムを分子レベルで解明中である。
