ヒト口腔上皮細胞の口腔粘膜免疫調節機構と歯周病

研究概要

本研究では、まず、ヒト口腔上皮細胞のインターロイキン(IL)-18の産生機構について検討した。IL-18はIL-12と相乗的に働きヘルパーT細胞を1型(Th1)にシフトさせる一方で、IL-12非存在下ではTh2サイトカインとして働く重要なサイトカインの一つである。その結果、(1)ヒト口腔上皮細胞はIL-18を24kDaの前駆体として細胞質内に発現すること、(2)さまざまな菌体表層成分やFas経路を刺激しても活性型IL-18は誘導されないこと、(3)インターフェロン-γで前処理した口腔上皮細胞を好中球プロテアーゼと内毒素性リポ多糖(LPS)との共存下で刺激した場合に活性型IL-18産生されるという知見を得た。
また、口腔上皮細胞は腸管上皮細胞と同様に、細菌パターン認識レセプターであるCD14分子の発現は認められないが、各種菌体成分認識にかかわるToll-like receptors(TLRs)/MD-2/MyD88系の分子群を発現している。にもかかわらず、腸管上皮細胞とは異なり、遊離型CD14の存在下でも多くの菌体成分に対して不応答性であった。これは、多種多様な微生物に常に晒されている口腔上皮細胞にとっては理にかなった現象と考えられる。一方、口腔上皮細胞は黒色色素産生菌(BPB)の菌体成分(綿毛や糖タンパク)に対しては応答し、顆粒球コロニー刺激因子や顆粒球マクロファージコロニー刺激因子を産生するという知見を得た。このことは、口腔上皮細胞が、炎症局所において顆粒球や単球の集積・活性化に関わる重要な役割を演じていることを示唆している。
さらに、BPBの糖タンパクはLPSとは異なり、TLR-2を介して宿主細胞を活性化するとの知見も得た。おそらく、BPBのLPS画分の特異な活性はLPS自体ではなく、標品に混入した微量のBPB特異的な糖蛋白様物資に帰因すると考えられる。
