再植後の歯髄・歯根膜再生過程における熱ショック蛋白Hsp25の役割に関する研究

研究概要

再植後の歯髄・歯根膜再生過程における低分子Hspの役割を明らかにする目的で、ラットを用いた再植モデルによりHsp25発現を検索し、その機能的意義を検討した。
動物は歯根未完成の生後4週齢ラットを用い、上顎第1臼歯を抜去後直ちに再植した。再植後1〜90日に灌流固定・脱灰し、パラフィン切片にはH-EおよびPAS染色、凍結切片には抗PGP9.5抗体、ED1抗体、OX6抗体、Hsp25抗体による免疫染色を行った。一部の試料は、免疫染色後に透過電顕の試料を作製した。
再植後1日では、歯髄内にOX6<+)ならびにED1(+)細胞の増加がみられたが、象牙芽細胞のHsp25発現が消失し、乱れた象牙芽細胞層内にはPAS(+)好中球浸潤が観察された。歯髄再生群では、3日後にOX6(+)ならびにED1(+)細胞が歯髄・象牙境に集積し、細胞突起を象牙細管の中に侵入させていた。これらの細胞はその微細構造から抗原提示細胞と思われた。5日後には歯冠歯髄でPGP9.5陽性神経の再生が進むと同時に、歯髄・象牙境には核小体の明瞭な細胞質の豊富な細胞が並び、Hsp251陽性を示した。これらの細胞は発達したゴルジ装置と粗面小胞体を有し、再生i象牙芽細胞であると考えられ、同細胞がHsp25発現を獲得することが明らかとなった。7日後には修復象牙質形成が開始し、OX6(+)ならびにED1(+)細胞の数が減少した。5〜7日後には歯髄内の再生神経やシュワン細胞がHsp25陽性を示し、神経の再生にもHsp25が重要な役割を示すことが示唆された。今回の結果より、再植歯において、抗原提示細胞が初期免疫応答に関与し、炎症をコントロールすることにより、歯髄再生過程に重要な役割を果たしていると考えられた。また、抗原提示細胞の局所への遊走、活性化に関与しているといわれるストレス蛋白を高濃度にもつ象牙芽細胞の損傷が、局所の免疫反応を賦活化し、迅速な歯髄再生過程を促していると考えられた。
