網赤血球測定装置を用いた破砕赤血球定量測定法の確立

研究概要	【当初の研究計画】自動血球計数装置(XE2100)の網赤血球測定チャンネルを用いて破砕赤血球を定量するプログラムを作成し、臨床応用の可能性を検討することを目的とした。【研究経過】赤血球に熱処理を加え破砕赤血球を作成し、プログラムを構築した。さらに鉄欠乏性貧血など小型の赤血球が出現する病態での偽陽性を防止するため、補正プログラムを作成し臨床応用の可能性を検討した。【研究成果】 (1)自動網赤血球測定チャンネルでは、ポリメチン系色素で染色された末梢血をフローサイトメトリ法で分析する。網赤血球測定チャンネルのスキャッタグラムにおいて小型赤血球を破砕赤血球比率と考え『Gate 1のdot数/赤血球+網赤血球』とし、目視法と相関するGate 1を設定した(r=0.986)。臨床検体で顕微鏡下に破砕赤血球が認められ、MCVが正常であった90検体でも、r=0.876,P<0.001と良好な相関を得た。小型赤血球の多い17検体では、小型赤血球がGate 1に混入するため、目視法と大きく乖離した。そこでGate2を設定し、そこに含まれる赤血球からGate 1の赤血球データを補正する方法を考案し次式を得た。式;補正破砕赤血球比率=測定破砕赤血球比率/e^<0.43Gate2/total> (2)溶血性尿毒症症候群、血栓性血小板減少性紫斑病症例、計14例について、本法が有用か否かを検討した。入院時のLDとは相関せず(r=0.512,P=0.061)、そのアイソザイムパターンを検討したところ、溶血のみによるLDの上昇ではなく、原因不明のLDアイソザイム5の上昇が認められた。そこで、破砕赤血球比率と、LD1+2の関係を検討したところ、有意な正の相関がみられた(r=0.732,P=0.008)。このことは、本法で測定した破砕赤血球が、溶血の程度を反映していると考えられた。
