新生児の非侵襲的脳機能画像計測システムを用いた発達的脳機能評価の基礎的検討

研究概要

(目的)新生児期の脳機能の発達的変化を検討するため近赤外光測定装置を用い、光刺激での視覚野の脳血液量や酸素化の特徴的変化を研究してきた。そして早産児の反応パターンは成人と異なり、特に光刺激での視覚野の脳血液量の減少を認め、この反応は発達により変化すると考えられた。今回は光刺激での視覚野の脳血液量減少の要因を調べる目的に正常成人を対象に光刺激の強度を変化させた検討を行った。また前年度に引き続き、成熟新生児を対象に光刺激での視覚野の反応を検討した。
(対象および方法)1対象は正常成人7例で、測定は多チャンネル近赤外光測定装置(OMM-2000島津製作所製)を使用し、後頭隆起を中心に24chのプローブを装着した。座位で安静閉眼時に、8Hzのフラッシュ光を眼前と100cm離して照らした。60秒毎に15秒間の刺激を7回行い光強度による違いを検討した。2成熟新生児10例を対象に仰臥位で自然睡眠時に、24ch光トポグラフィー(日立メディコ社製)を用いて8Hzのフラッシュ光を照らし、後頭葉で同様な計測を行った。全症例で両親または本人の承諾を得た上で検査を行った。
(結果および考案)1 成人においては光強度が弱い時には脳血液量が増加し、強い時は脳血液量が減少するパターンも認められたが、その傾向や程度は様々であった。2 成熟新生児においては、測定装置を変えても早産児と同様に光刺激により視覚野での反応を認め、刺激時には脳血液量は一過性に増加し、刺激終了時には前値より有意に低下し以後前値に回復した。この反応は成人の脳血液量が増加するパターンと異なっていた。以上の結果より、成熟新生児においても光刺激での視覚野の脳血液量反応パターンが成人と異なることが実証された。しかし更なる脳血液量減少の要因の検討として、反応個所の差異や特に覚醒状態の評価、他の脳機能モニターとの比較などの検討が必要と考えられた。
