スポーツ活動における肘関節骨軟骨障害に関する生体力学的検討

研究概要

(臨床的観測)肘頭の形態と各種肘頭過労性骨障害の出現率との関係を検討したところ、肘頭疲労骨折では肘頭が台形のものに、いわゆるVEO(vulgas extension overload injury)では尺側凸のものに、疲労骨折類似型ではドーム状のものに多発することを確認した。さらに、肘頭および肘頭窩の形態と肘頭過労性骨障害との関連について検討した結果、肘頭が尺側凸型で肘頭窩が外側陥凹型では60%、肘頭が尺側凸型で肘頭窩が肘頭陥凹型では28.6%、肘頭が台形型で肘頭窩が外側陥凹型では22.2%で障害を認めた。以上より、疲労による肘頭障害は、肘頭自身の形態に加え、相対する肘頭窩の形態とのコンビネーションにも大きく影響されることが分かった。
(有限要素解析)33歳男性の肘関節を1mm間隔でCTを撮影し、Mark Mentat softwareを使用して、コンピューター上で、3次元にて再構成した。作製した有限要素モデルに外反伸展負荷を与え、肘頭での応力集中点をVon Misesの解析を用い検討した。その結果、台形型では疲労骨折と同様な部位に応力集中を示し、尺側凸型では肘頭尺側先端にのみ応力集中を認め、いわゆるVEO発生部位に一致した。ドーム状の肘頭では肘頭全体に一応な応力集中を示した。有限要素からみた理論的疲労障害発生メカニズムの検討からも、各種疲労障害発現には、肘頭の形態が重要であることを示している。
