前立腺癌罹患の危険率予測に関するニューラルネットワークの構築

研究概要

前立腺癌スクリーニングにおいて、現在、前立腺特異抗原(PSA)は診断精度が高く、中心的役割を担っている。本研究では、検診でPSAを導入する以前の1988年〜1991年の凍結保存血清のPSA値を測定し、初回検診受診時のPSA値・年齢と将来の前立腺癌罹患の危険因子について検討し、適切なスクリーニングプログラムの提案を行うことを目的としている。PSA値は、全自動エンザイムイムノアッセイ装置AIA600II(東ソー社製)を用いてE-test Tosoh II PAキットによる測定をおこなった。検診時のPSA値が既知の1993年度の凍結保存血清90例を用いてPSAの再測定をおこなった。20℃での長期の凍結保存により血清PSA値の免疫学的な活性は、PSA2-10ng/mlの範囲では、10年間の保存期間で90.7±6.2%(平均±S.D.)低下していた。つづいて、1988年〜1990年の3年間に前立腺癌検診を受診した1,529例のPSA値の測定を行った。その結果、全症例のPSA値(補正後)は0.0〜84.9ng/mlで中央値は0.76ng/ml、平均±標準偏差は1.6±3.7ng/mlであった。初回検診受診者1,265名に中で、初診時のPSA値が4.0ng/ml以下の症例は1,181名おり、PSA値別には0.0-1.0ng/mlは764例、1.1-2.0ng/mlは258例、2.1-4.0ng/mlは159例であった。年齢別には49歳以下は43例、50-59歳は172例、60-69歳は568例、70-79歳は338例、80歳以上は60例であった。各PSA値・年齢別の将来の前立腺癌発見率は、PSA値0.0-1.0ng/mlは0.26%(2/764)、1.1-2.0ng/mlは0.78%(2/258)、2.1-4.0ng/mlは8.2%(13/159)であった。年齢別には49歳以下は0%(0/43)、50-59歳は1.2%(2/172)、60-69歳は1.6%(9/568)、70-79歳は1.5%(5/338)、80歳以上は1.7%(1/60)であった。初回受診時のPSA値が4.0ng/ml以下の症例において、年齢は将来の癌危険因子にはならず、初診時のPSA値は予測因子として重要であり、検診受診間隔の設定において重要な指標になると予測された。
