リンパ管増殖とリンパ節転移の分子機構の解明と臨床応用に関する研究

研究概要

がんにおけるリンパ管新生の分子機構を解明するために、受精鶏卵chorioallantoic membrane(CAM)にrecombinat VEGF-Cを作用させ、リンパ管の新生過程を形態学的に検討した。また人胃癌組織におけるVEGF-C, VEGF-Dおよびその受容体であるVEGFR-2, VEGFR-3の発現をRT-PCR, Western blot, 免疫染色でしらべた。
結果1)recombinat VEGF-Cは受精鶏卵CAMに4日目でリンパ管および血管を新生させた。電子顕微鏡でみると基底膜のない幼弱なリンパ管はCAMの間質に新生していた。またCAMの上にヒト胃癌継代培養細胞株6株をのせ培養後、BrdUで標識し、抗BrdU抗体で免疫染色すると、リンパ管内皮細胞の標識像が散見された。さらにVEGF-C産生株では有意にリンパ管数の増加が認められた。新生リンパ管はCAMと癌との境界部に多数みられ、その部位では血管の数も増加していた。
結果2)ヒト胃癌継代培養細胞株8株ではNKPS, TMK-1, AZ521, MKN-45でVEGF-C mRNAの産生がみられた。一方、VEGF-D mRNAはKATOIII, MKN-7で発現しており、VEGF-C mRNAとは逆の発現パターンを示した。VEGF-C, VEGF-Dの受容体であるVEGFR-3mRNAは胃癌ではKKLSで発現しているのみであった。臨床材料でみてみるとVEGF-C,-Dはがん組織から高率に発現がみられたが、その受容体であるVEGFR-3 mRNAは胃癌組織でも正常粘膜でも発現を認めた。免疫組織学的にみてみるとVEGF-C, VEGF-Dはがん細胞の細胞質に局在しているが、正常粘膜や間質の線維芽細胞でも発現がみられた。一方、VEGFR-3は胃癌組織や正常粘膜のリンパ管内皮細胞に限局して発現していた。とくにVEGF-CおよびVEGF-Dを発現している胃癌組織ではVEGFR-3陽性のリンパ管数が有意に増加していた。さらに注目すべき点はVEGF-C, VEGF-Dともに産生している胃癌ではいずれも産生していない癌にくらべリンパ管数が増加していただけでなく、リンパ節転移も有意に多く出現した。
以上の事実からVEGF-Cはリンパ管新生に重要なかかわりを有しており、癌組織のVEGFR-3陽性のリンパ管を増加させ、結果的にリンパ節転移増加するものと考えられた。
