研究概要	アトピー性皮膚炎atopic dermatitis(AD)は乳幼児期に発症が始まる慢性炎症性皮膚疾患である。遺伝性素因にアレルゲン感作、細菌感染が加わっておこると考えられているが、その発症機序に関しては不明な点が多い。これ迄の研究で我々は皮膚ケラチノサイトでIL-18の産生を過剰にすると、多クローン性のIgE産生とAD様の慢性掻痒性皮膚炎が発症することを示してきた(P.N.A.S.,2002)そこで、平成15年度の研究では、病原体成分で上皮細胞を刺激し、IL-18の産生を誘導するとともに、その様な処理を受けたマウスの血中でIL-18とIgEの両者の値が高値になるか検討した結果、IL-18が上昇しIgE産生が同時におこることを明らかにした(Int Immunol,2003)。病原体成分としてprotein Aを用いて皮膚刺激した。その理由は、AD罹患児は黄色ブドウ球菌などの化膿菌に頻繁に感染するからであり、特に黄色ブドウ球菌の中のCowan-I株がprotein Aを産生するからである。平成15年度にはまた、IL-18によるIgE産生誘導メカニズムを研究した。IL-18をC57BL/6バックのCD1欠損マウス(NKT細胞は欠損するが、コンベンショナルCD4^+T細胞は保有する)あるいはクラスII欠損マウス(NKT細胞は保有するが、コンベンショナルCD4^+T細胞は欠損している)に投与してもIgE産生が誘導されないが、クラスII欠損マウスにコンベンショナルCD4^+T細胞を移入すると、野生型と同様に、IL-18を投与されると、IgE産生能を示すことを明らかにした(J Exp Med,2003)。更にまた、一ヶ月前にメモリー型Th1細胞を受身移入されたナイーブマウスに、経鼻的に抗原とIL-18を投与すると、Th1、Th2、ケモカインが産生され、気道の炎症と過敏症が誘導されることも明らかにした(J Exp Med,2004)。以上、IL-18がアレルギー性炎症の誘導に深く関わることを示してきた。
