アディポネクチンの2型糖尿病・高脂血症・動脈硬化症における病態生理学的の解明

研究概要

本研究では、アディポネクチンによる糖・脂質代謝、動脈硬化制御メカニズムを明らかにすることを試みた。アディポネクチンヘテロ欠損マウスで、普通食下でもインスリン抵抗性を認め、ホモ欠損マウスではより強いインスリン抵抗性・耐糖能障害を認めた。これらの表現型は高脂肪食負荷でさらに増悪した(J.Biol.Chem.277:25863,2002)。著明な痩せと良好なインスリン感受性を示すCBPヘテロ欠損マウスでは、レプチン作用及びアディポネクチンレベルの著明な増加を認めた(Nature Genetics 30:221,2002)。アディポネクチン過剰発現ob/obマウスではPPARαが活性化され、脂肪酸燃焼などが促進されて、組織内中性脂肪含量が低下し、高FFA血症・インスリン抵抗性・糖尿病の改善が認められたが、肥満の改善は認められなかった(J.Biol.Chem.278:2461,2003)。優性抑制型AMPKを用いた検討より、アディポネクチンはAMPK活性化を介して骨格筋で脂肪酸燃焼、糖取り込みを促進し、肝臓で糖新生を抑制し、血糖を低下させることが示された(Nature Medicne.8:1288,2002)。アディポネクチン過剰発現apoE欠損マウスは、動脈硬化巣の形成が抑制されており、スカベンジャー受容体AやTNFαの発現の抑制が認められた(J.Biol.Chem.278:2461,2003)。これら遺伝子操作マウスを用いた解析より、我々はアディポネクチンが急性にはAMPキナーゼを、慢性にはPPARαを活性化し、脂肪酸燃焼などを促進して、組織内中性脂肪含量を低下させて、インスリン抵抗性を改善させていることを見い出した。またアデイポネクチンは個体レベルでの生理的なインスリン感受性、糖・脂質代謝の重要な調節因子であると共に、直接的な抗動脈硬化作用を有することが示された。その補充は直接・間接に生活習慣病を改善させることが示された。
