生体臓器移植におけるドナー特異的免疫寛容誘導の臨床応用へのアプローチ

研究概要

今年度の実験はマウスを使用して行われた。組み合わせはドナーがCBA,レシピエントがC57BL/10でMHCが完全に異なる組み合わせである。無処置のレシピエントはドナーの心臓を8日で拒絶する。一方、移植7日前にドナーの脾細胞を気管内に投与すると移植心は全て30日以上生着し約30%は100日以上生着する。拒絶日の中間値は60〜80日である(約20回の実験群すべてにおいて)。かつドナーの脾細胞を経気管内投与し7日後に、心臓移植ではなく、そのレシピエントの脾細胞を分離し、脾細胞を無処置のCBAレシピエントに移入する。同日、C57BL/10の心臓を移植すると、その心臓は中間値で約60日生着する。このことは、免疫制御細胞がドナーの気管内投与後7日で誘導されたことを意味する。2003年度に得られた研究結果は以下である。
(1)免疫制御細胞の誘導を増強するには、CD40経路やCD4経路の遮断が極めて有効である。(Transplantation. 2003 ; 75(6) : 878-84., Transplantation. 2003 ; 76(9) : 1305-14.)
(2)免疫制御細胞の誘導には、CTLA4経路、PDL-1経路、CD27経路、CD134経路、CD30経路がそれぞれに必要である。(Transplantation. 2004 ; 77(1) : 6-12., Transplantation. 2003 ; 76(5) : 772-6., Transplantation. ; 75(10) : 1636-9.)
(3)免疫制御細胞は、メモリー細胞も抑制する。(J Thorac Cardiovasc Surg. 2003 ; 126(3) : 853-4)
現在進行中の実験結果で、投稿中および投稿準備中の結果は以下である。
(1)免疫制御細胞の誘導および機能には、サイトカインレベルでは、TGF-betaではなく、IL-10が必要である。
(2)ドナー抗原は間接経路で抗原提示されている。
(3)免疫制御細胞は、CD4(+)CD25(+)細胞の他、樹状細胞自身も候補である。
(4)免疫制御細胞の誘導には、胸腺は不要であるが、胸腺からでた若いTリンパ球が必要である。
