市中獲得MRSAの消長の経時的観察とその病原性の調査研究

研究概要

平成13年7〜9月および平成14年3月の2回、東北地方の幼稚園および保育園(計4施設)を対象とし、一般健康児とその職員におけるMRSA鼻腔保菌状況を検討した。第1回調査では、児童364名/職員39名を対象とし、第2回調査は、児童294名/職員21名を対象とした。第1回調査では、22名5.5%(児童19名/職員3名)、第2回調査では12名3.8%(児童11名/職員1名)からMRSAが検出され、高い保菌率を示した。次いで、分離されたMRSAを、コアグラーゼ型、SCCmecのタイプ、MLSTなどのgeno typeを検討し、病院由来のMRSAと比較した。これらの検出されたMRSAの多くはコアグラーゼ1型(第1回調査/第2回:14株/9株)であり、次いでコアグラーゼ2型(7株/2株)であった。コアグラーゼ2型MRSAは、薬剤感受性、SCCmec typing、MLSTによる疫学的解析が、院内獲得型MRSAと同等であり、病院からの流出が考えられた。一方、本検討にて見いだされたコアグラーゼ1型MRSAは、βラクタム薬にヘテロ耐性を示すほか、院内獲得型MRSAから主に検出されるTypeII-SCCmecと構造が一部異なる、新規のTypeII subtype b-SCCmec(現在、塩基配列を決定中)を保有することが判明し、MLSTによる疫学的解析の結果、新規のST89に属することが明らかとなった。また、PFGEによる疫学的解析の結果では、各施設間では、別々のパターンが集約する傾向がみられ、施設内での水平感染による流布が示唆された。これらの結果により、日本の一般社会においても、すでに市中獲得型MRSAが存在すること、また、海外から報告されるTypeIV-SCCmecを保有する市中獲得型MRSAとは由来が異なる日本独自の市中獲得型MRSAであることが強く示唆された。ひきつづき、経時的調査を行い、その動向を検討していく予定である。
