膵液中の癌関連遺伝子異常を指標とした膵癌の早期診断に関する研究

研究概要	内視鏡下経乳頭的に採取した膵液において、mesothelin mRNAの発現を検討した。膵液中mesothelin mRNAの陽性率は膵癌で52%(11/12)、膵管内乳頭状粘液性腫瘍(IPMT:過形成か腺腫)で45%(5/11)、慢性膵炎で14%(3/22)であった。但し、慢性膵炎やIPMTでは膵癌に比してmesothelin mRNAの出現強度は弱い場合が多かった。また、その陽性率は膵癌の腫瘍径や病期分類との間には関連性は認められず、小膵癌や早期膵癌にも陽性例がみられた。一方,過形成や腺腫であったIPMTにもかなりの陽性例がみられ、膵腫瘍の良悪性の鑑別診断には問題は残すが,今後定量的な検討にてこのような問題は克服できると思われる。同様に膵液においてppENK(proproenkephalin)のプロモーター領域のメチル化異常について検索し、膵癌診断における有用性を検討した。ppENKのメチル化は膵癌で41%(7/17)で認められたが、慢性膵炎15例ではいずれもメチル化は認められなかった。なお、メチル化が認められた膵癌7例中p53変異を検索した症例が2例あり、うち1例はp53変異は陰性であった。以上より、ppENKのメチル化異常は膵癌に対する特異性が高く、p53変異陰性例にも検出し得る事が判明した。 Real-time PCR法によりhTERT(human Telomerase Reverse Transcriptase)mRNAの定量的測定法を確立し、その定量性は0.33pg〜30ngで得られる高感度なものであった。健常者の血清hTERT mRNAは19例中6例で検出されず、13例でhTERT/18Sは3.5x10^<-4>であった。一方、膵癌では全例hTERT mRNAが検出され、hTERT/18Sが3.5x10^<-4>より高い例が5例(40%)にみられた。血清hTERT mRNAの測定は膵癌の診断に有用なことが示唆された。
