肝細胞癌制圧をめざした移植医療確立のための総合的研究

研究概要

平成16年2月までに成人38例に対して、生体部分肝移植を37例に、脳死肝移植1例に行った。そのうち、肝細胞癌症例は14例であった。全例術後1ヶ月から24ヶ月経過している。14例中2例に再発(骨1例、肺1例)を認めているが、全例生存中である。14例中10例がこれまで肝細胞癌症例に対する肝移植の適応基準とされてきたミラノ分類の適応外症例であった。ミラノ分類以外に新たな肝移植後の肝細胞癌再発予測の適応基準を探るため、全身に散布された肝細胞癌細胞が既に存在し、それが術後の再発の原因となり得るかの検討を行った。移植例全例から移植前に末梢血を採取し、Light CyclerTMを用いたReal Time PCR法によるAFPmRNA発現を指標とした遺伝子診断を行なった。その結果、4例が陽性であり、10例は陰性であった。陽性4例のうち2例が再発例であった。また、再発例の2例はミラノ分類逸脱例であった。また、術中散布された癌細胞の生着を予防するために、ミラノ分類逸脱の10例に対して、補助療法として移植手術時の無肝期に抗癌剤を全身静脈内投与した。抗癌剤投与による周術期の大きな合併症は認められなかったが、再発の2例も投与例であった。また、肝細胞癌の再発危険因子を探索するために、肝切除材料を用いた、再発予測の生物学的な予後因子の検索を行うと共に、網羅的遺伝子発現解析の手法を用いて、ウィルス肝炎合併肝癌の遺伝子プロファイリングや肝炎ウィルス感染肝癌移植例の肝生検材料を用いた拒絶反応とウィルス肝炎の再発の病態の違いを針肝生検材料で網羅的遺伝子発現解析可能とする条件設定を行い、施設内倫理委員会の承諾の上、解析を行った。
