自己免疫疾患の病態規定因子の解明と予後診断法の開発

研究概要

自己免疫疾患の予後診断法を開発するため、免疫調節細胞(CD25+CD4+細胞、CD8+樹状細胞、NKT細胞、CD30+CD8細胞、等)が自己免疫疾患の病態・予後を規定する因子である可能性を調べた。
自己免疫性甲状腺疾患の標的臓器(甲状腺)に浸潤している(特異的な)免疫細胞を分離し、免疫調節細胞(CD25+CD4+細胞、CD8+樹状細胞、NKT細胞、CD30+CD8細胞、等)の数、比率、大きさ、細胞膜分子の発現強度をフローサイトメトリーで解析した。そして、末梢血に存在する(非特異的な)免疫調節細胞と比較した。さらに、種々の病態における末梢血免疫担当細胞の動態を調べることにより、病態と病因への関連を調べた。結果は以下の通りであった。
1.CD25+CD4+細胞の比率は、末梢血と甲状腺内で差はなかったが、活性化CD4+細胞の比率が甲状腺内で増加していることより、調節性CD25+CD4+細胞の比率が甲状腺内で低下していることが示唆された。
2.自己免疫応答を抑制するNKT細胞の比率は、末梢血に比し甲状腺内で低下していた。
3.甲状腺内FasL+CD4+CD8+樹状細胞比率の著明な増加とCD4+T細胞の比率の低下およびFas発現の増強より、甲状腺内でFasL+CD4+CD8+樹状細胞がFas+CD4+Tリンパ球のアポトーシスを誘導している可能性が示唆された。
4.永続性甲状腺機能低下症でT4補充療法中の橋本病において、自己免疫および細胞傷害機序を抑制するCD30+CD8+細胞上のCD30分子の発現強度が低下していた。
5.甲状腺上皮細胞には、以前の報告とは異なり、甲状腺浸潤リンパ球に見られるような明瞭かつ強度なFasLの発現は認められなかった。
