ヘリコバクタ・ピロリ感染による胃癌発症に影響する遺伝因子、環境因子の同定―アジア各国を中心とする遺伝因子、環境因子の比較研究―

研究概要	アジア諸国でのヘリコバクタ・ピロリ(HP)感染率はおしなべて高率であるが、それによる上部消化管疾患の発症率は大きく異なる。その原因をHPの菌種の差で説明しようとする意見もあるが、明らかな差異は見出されていない。そこで本研究では特にHP感染による胃癌の発症に影響すると考えられる、宿主因子、環境因子を明らかにする目的で、アジア諸国のHP感染患者を対象に種々の調査研究をおこなって、各国間で比較した。まずHP感染率は各国間で大きな差はなく、40-50%であった。つぎに血清ガストリン、PGI, PGII, PGI/II比については、HP陰性者では中国南部、北部、韓国、日本、タイの間で差はなかった。一方HP感染群では、ガストリンは日本が最も高く、韓国、中国北部の順で、中国南部、タイでは軽度の増加にとどまっていた。さちにPGI/II比は、やはり日本でもっとも低く、韓国、中国北部、南部、タイの順に低下していた。さらにHP感染者の内視鏡検査では、日本、韓国、中国北部、南部、タイの順に胃体部胃炎、萎縮性胃炎が著明であった。つぎに胃酸分泌テストにおける最高酸分泌量(MAO)はHP非感染者では日本、韓国人の腎酸分泌は、中国、タイ人のそれよりも低かった。さらにHP感染者では、日本、韓国、中国北部、南部、タイの順に胃酸分泌は低下していた。一方、食習慣などのサーベイランスにおいて、現在のところ日本、韓国では中国、タイに比べて喫煙率が明らかに高いこと、さらに食塩摂取が明らかに高いことが判明した。以上の成績から、同じHPに感染しても、日本人、韓国人では萎縮性胃炎に移行しやすいこと、さらにその要因として、本来の胃酸分泌が低いこと、さらに食塩や喫煙などの食習慣、生活習慣の関与か考えられた。
