抗リン脂質抗体症候群におけるマクロファージの役割:抗原提示および血栓形成の機序

研究概要	「抗リン脂質抗体症候群(APS)」は、自己抗体による易血栓性が基本病態の自己免疫疾患である。申請者らは、本疾患で出現する抗β2-グリコプロテイン1(β2-GPl)抗体と酸化LDL・β2-GPI複合体との免疫複合体がマクロファージに取り込まれ、泡沫化が起こることを示してきた。本年度の研究では、APS患者末梢血リンパ球より樹立したβ2-GPIに特異的T細胞クローンと健常人由来の樹状細胞を用いて、β2-GPIと酸化LDLリガンドやフォスファチジルセリンを含有するリボソームにより起こる抗体非依存的な抗原提示能(antigen-presenting activity, APC活性)について解析した。方法としては、各種リボソーム、β2-GPI、およびIL-2などのリンフォカイン存在下で、放射線照射した樹状細胞およびβ2-GPI特異的T細胞を培養し、^3H-TdRの取り込み量を測定しAPC活性の指標とした。その結果、APC活性は、β2-GPI単独の添加では現れず、β2-GPIと酸化LDL由来のリガンド(oxLig-1、7-ketocholesteryl-9-carboxynonanoate)含有リボソーム、牛脳由来のフォスファチジルセリン含有リボソームなどとの共存下で有意に認められた。APC活性が現れるリボソームの何れもがβ2-GPIに対する親和性を有する酸性脂質を含有していた。すなわち、マクロファージや樹状細胞にβ2-GPIが抗体非依存的に取り込まれ、抗原提示を促すには、β2-GPIが、これらの細胞に認識される酸性脂質含有リボソームと複合体を形成する必要があることが明らかになった。酸化LDLもβ2-GPIの取り込みを促す脂質粒子である。本年度、酸化LDLとβ2-GPIの相互作用について詳細な解析も行った。その結果、酸化LDLとβ2-GPIは共存すると速やかに静電的作用で結合する。さらに37℃数時間のインキュベーションで、Schiff-base adductを形成しstableな複合体となる。酸化ストレスが発症に関与すると考えられている様々な疾患例えば、糖尿病、慢性腎炎やSLE、APSでも、高率に、この種のstableな複合体が血中に出現していることが明らかとなった。APSにおいては、この種の脂質・β2-GP1複合体が、自己抗原としてマクロファージや樹状細胞による抗原提示を誘導する可能性が示された。
