潰瘍性大腸炎の癌化における間質細胞の遺伝子不安定性・変異の果たす役割

研究概要	潰瘍性大腸炎(UC)の癌化における間質細胞の遺伝子不安定性・変異を検索すべく、1、その対照として特発性大腸癌40症例の癌病変と非癌大腸粘膜を選び、薄切切片上におけるmicrodissection法によって上皮細胞と間質細胞とを分離採取し、DNA抽出・PCR-SSCP法によりmicrosatellite instability (MSI)を検索した。その結果、癌組織では上皮、間質細胞にMSIが34,41%に認められ、上皮細胞では癌細胞の分化、Dukes stageの進行に相関していたが、間質細胞では高分化腺癌(54%)、DukesA(55%)と高く、逆の相関であった。更に上皮細胞のp53蛋白発現と間質細胞のMSIとは有意に逆相関が認められた。このことから、大腸癌発生において間質細胞の関与を含む新たなる発癌機構の存在が示唆された。続いて少数例ながら、2、UCに伴う異型病変、癌病変の解析を同様の方法で行い、上皮細胞のMSIないしLOHは再生上皮、異型病変、癌と徐々に上昇していたが、間質細胞では再生上皮の段階から既に高率(約50%)に認められた。以上から、UCにおける腫瘍性病変では、間質細胞の遺伝子不安定性は再生性病変から癌病変まで一定して高率に生じているが、上皮細胞は病変の進行に伴ってMSI, LOHの出現率が上昇していることが判明し、特発性大腸癌との発癌機構の違いが明らかになった。今後の研究:大腸腺腫、UCにおける腫瘍性病変症例を多数検索することにより、UCに伴う大腸癌の発癌機構の特異性を間質細胞の遺伝子不安定性、変異の面から明確にする。
