B型慢性肝炎に対する抗ウイルス療法とHBV遺伝子変異との関連についての研究

研究概要

B型慢性肝炎患者に対し、ラミブジン治療を行って、その治療効果、治療中のウイルスの再上昇(breakthrough hepatitis)、また治療終了後のALTの再燃(post treatment hepatitis)について、各HBV genotype群に症例を分けてその影響を検討することを目的とした。現在までにラミブジンを投与された40症例について、その治療前ウイルス量をreal time PCRにて定量し、また投与12ヶ月目の検体については、polymerase遺伝子内の逆転写酵素活性部位アミノ酸YMDDの変異の有無をdirect sequenceにて判定し、全体としてラミブジンの治療効果やbreakthrough hepatitisの有無との関連について検討した。まずgenotypeとの関連を見ると、genotype Bと較べgenotype C型患者ではHBe抗原の低下率やHBV DNA量の低下が不良であったが、ALTの正常化率は同等であった。また治療前HBV DNA量はgenotype BとCの患者間では差はみられなかった。YMDDの変異は、HBV DNAの低下が不良の例、あるいはALTの治療中の再上昇の例に多く見られた。また、gentoype B型の症例では、多くの例で治療を終了してもALTの再上昇が見られた例が少なく、いわゆるpost treatment hepatitisはgenotype B型症例で有意に少なかった。現在さらに、genotype B型およびC型症例におけるbreakthrough hepatitis症例について、その治療前と12ヶ月目の検体におけるウイルスクローンの変化について検討を進めている。
