クローン病の肉芽腫における疾患特異抗原へのT/NKT細胞活性化の機序と意義

研究概要

クローン病では、マクロファージにおけるNOD2等、腸内細菌などに対する抗原提示細胞の免疫応答を規定する遺伝子のSNPなどの異常を背景とし、腸管内に存在する限定された疾患関連抗原に対するT細胞系の免疫応答が患のinitiation重要であると考えられている。これまでの研究によりクローン病の非乾酪性肉芽腫が、そのような抗原への抗原提示細胞の異常な応答とT細胞活性化の場であることが明かとなった。さらに本症では、このような免疫応答に脂質抗原の関与が想定されるため、肉芽腫性病変形成部へのマクロファージ系細胞の種属機序を、ホーミングレセプターから解析した。
1)健常腸管粘膜では、粘膜筋板よりにMAdCAM-1陽性血管内皮が少数みられた。
2)クローン病および、潰瘍性大腸炎では活動性炎症のみられる粘膜固有層にMAdCAM-1陽性血管内皮がみられた。
3)クローン病では、肉芽腫および所属リンパ節にMAdCAM-1陽性血管内皮の増加がみられ、その周囲にCD3陽性のT細胞の集簇がみられた。
4)これらのT細胞は、Ki67陽性で増殖期にある活性化T細胞が主体であった。
5)同部位には、CD138陽性の樹状細胞の増加がみられ、MAdCAM-1陽性血管内皮を介して、ホーミングしたT細胞に対して抗原特異的な免疫応答を惹起していることが考えられた。
以上より、クローン病では、MAdCAM-1によりホーミングの制御を受ける粘膜免疫型T細胞の抗原特異的活性化が認められ、この抗原が病因に密接に関与したものであると考えられた。
