心肥大・血管新生の機序解明と治療的電気刺激血管新生法の確立と応用

研究概要

ラットの両側大腿動脈を切離し虚血肢モデルを作成した。一週間の回復期間を与えた後、電極(陰極)を両側前脛骨筋中央の筋肉内に留置し、肩甲骨下より出した。右側の前脛骨筋に留置した電極をパルスジェネレーターに接続し電気刺激肢とした。左側の前脛骨筋に留置した電極をパルスジェネレーターに接続せず非刺激肢とした。
1)5日間刺激の血管新生因子の効果:continuous LVES(CL-VES)、intermittent HVES(IH-VES)の2条件間の比較.
筋収縮閾値以下の電気刺激(CL-VES)が虚血骨格筋の血管新生因子であるVEGF、HGF、FGF、あるいは炎症・低酸素因子であるIL6、HIFに及ぼす影響について検討した。さらに収縮性電気刺激(IH-VES)との比較を行った。CL-VESではHGF、VEGFの増加を認めたが、IL6、HIF、FGFに関しては差異を認めなかった。IH-VESではVEGFの増加を認めたがHGFの増加を認めなかった。虚血肢に対するCL-VESは、低酸素や炎症を介さずにVEGF・HGFを増加させることにより血管新生に関与し,その機序はIH-VESとは異なる可熊性が示唆された。
2)1日間刺激の血管新生因子の効果:CL-VESとIH-VESの比較
CL-VES群において、電気刺激肢のVEGF蛋白量は非刺激肢に比較し有意に増加した。一方、IH-VES群において、電気刺激肢のVEGF蛋白量は非刺激肢と比較して差異を認めなかった。すなわち、1日間のラット骨格筋に対する電気刺激ではCL-VES群においてのみ非刺激肢と比べてVEGF蛋白量の統計学的有意差を認めた。一方、1日間刺激に対する5日間刺激のVEGF蛋白量/コントロール群のVEGF蛋白量の比率の増加はIH-VES群においてのみ認められ、CL-VES群においては認められなかった。
