循環調節因子アドレノメデュリンの病態における意義と治療薬、診断薬としての臨床応用

研究概要

申請者らは『循環調節因子アドレノメデュリンの病態における意義と治療薬、診断薬としての臨床応用』を目的として、各種の循環器疾患の病態モデルやヒトにおける臨床研究により、以下の研究業績を挙げた。
1.ラットの高血圧性心肥大モデルにおいて、アドレノメデュリンの遺伝子発現、その蛋白レベル、・アドレノメデュリンの構成蛋白のCRLR、RAMP2、3の遺伝子発現、その蛋白レベル、さらにアドレノメデュリンの最終段階の生合成過程に関与するアミド化酵素活性も亢進していることを示した。
2.ラットの高血圧性心肥大から心不全モデルを発症するモデルにおいて、アドレノメデュリンの遺伝子発現・蛋白レベル、アドレノメデュリンの構成蛋白のCRLR、RAMP2、3の遺伝子発現・蛋白レベル、さらにアドレノメデュリンの最終段階の生合成過程に関与するアミド化酵素活性は心肥大期に亢進し、不全心期には、アドレノメデュリンの遺伝子発現・蛋白レベル、およびアミド化酵素活性がさらに亢進することを示した。
3.ラットの高血圧性心肥大から心不全モデルを発症するモデルにおいて、アドレノメデュリンを長期投与すると、心重量、血行動態、神経体液性因子、心筋の遺伝子発現が改善することを示した。さらに内因性の心機能も改善することを示した。
4.ヒトの先天性心疾患において、血漿でアドレノメデュリンの2分子型が増加していることを明らかにし、その増加の原因には低酸素血漿が関与していることを示した。
5.さらにアドレノメデュリンと関連する作用を有するナトリウム利尿ペプチドが動脈効果抑制作用をもつこと、C型ナトリウム利尿ペプチドが心臓において産生される可能性を示した。
