遺伝子導入造血幹細胞の体内増幅を組み合わせた慢性肉芽腫症遺伝子治療

研究概要	現行の造血幹細胞遺伝子治療における主問題は、幹細胞への遺伝子導入効率の低い上、これが非遺伝子導入細胞で希釈されて治療効果が減ずることである。その対策として、遺伝子導入細胞を体内増幅するため、造血因子受容体に分子スイッチを付加したキメラ受容体の遺伝子(「選択的増幅遺伝子」)を開発している。対象疾患であるX連鎖型慢性肉芽腫症(X-CGD)は、gp91欠損により顆粒球・単球が活性酸素産生能を欠く遺伝病で、遺伝子ノックアウトにより作出されたモデルマウスを用いて前臨床研究を行った。昨年度は、顆粒球コロニー刺激因子受容体のシグナルをエストロゲン受容体で制御するキメラ蛋白質(GcRER)の有用性を示したが、今年度は、より投与しやすい薬剤であるエリスロポエチン(Epo)に反応するよう、Epo受容体とc-Mplを組み合わせたキメラ蛋白質(EpoRMpl)について検討した。ヒトgp91遺伝子、SAGとしてエリスロポエチン受容体細胞外部分とトロンボポエチン受容体細胞内部分のキメラ蛋白質(EpoRMpl)遺伝子を搭載するレトロウイルスベクターを構築し、X-CGDモデルマウスの骨髄細胞に両遺伝子を導入した。このドナー細胞を、致死量放射線照射したレシピエントX-CGDマウスに移植して造血系再構築後Epoを投与し、活性酸素産生能を回復した顆粒球の変動を定期的に追跡した。その結果、4週毎のEpo反復投与により、機能回復顆粒球の割合は最高27.5±6.8%(n=3)まで上昇し、非投与群8.4±3.1%(n=3)に対して有意に高くなった(P=0.01)。機能回復顆粒球の絶対数もEpo投与群で高い傾向が認められた(1349±1010/μl対268±82/μl)。以上から、EpoRMpl遺伝子導入によりCGDに対する治療効果が増大することを検証した。現在、Epo投与に反応するまでのタイムラグを短縮するため、新規キメラ分子の開発を進めている。
